In both studies, injection drug use was the most important risk factor, and the prevalence of hepatitis C virus infection was much higher than that of HIV infection. In a related commentary, Elliott discusses the continued lack of response by government policy-makers regarding the implementation of evidence-based harm-reduction strategies in prisons. **See pages** [252](http://www.cmaj.ca/lookup/volpage/177/252?iss=3)**,** [257](http://www.cmaj.ca/lookup/volpage/177/257?iss=3) **and** [262](http://www.cmaj.ca/lookup/volpage/177/262?iss=3) **Disclosing medical errors to patients** In this commentary, Levinson and Gallagher predict that disclosing medical errors to patients will soon be a routine part of medical care.
